These group of drugs block the uptake of folic acid by cells of the body, cells require folic acid to power the process of growth and multiplication, and once the uptake is blocked the cancerous cells will not be able to continue dividing and multiplying.
Pemetrexed can be combined with another anti cancer agent called carboplatin and this combination has been known to increase the life span of patients by as much as three years.
If you are a patient suffering from this cancer, and your oncologist has recommended the use of Pemetrexed as part of your cancer treatment, it is vital to obtain information about nutritional supplements that may be necessary during the course of chemotherapy. Clinical trials are generally bio-medic research studies in humans that strictly follow a pre-defined protocol. It is a process that all potential treatment alternatives and medicines go through before they are considered safe or effective for the general population.
People who participate in these trials are most often than not patients who have invasive and treatment-resistant diseases. Most of these patients see it as their last hope for a cure.
There are three stages.
Phase I
is for determining the right dosage and safety of a particular drug. This phase is not to test the efficacy of the drug, but more the safety. This phase is normally tested in a small group of people; usually 20-80 persons for the first time.
Phase II
is for determining if the new drug really works against a particular health problem. This stage further evaluates the optimum dosage and confirms the results of Phase I trials. Phase II gives tests to a larger group of people; around 100-300 persons.
Phase III
is the final phase of a clinical trial before the new drug is approved for treating a disease. In this stage, the new drug is compared against the current standard treatment for a particular illness. Different doses and drug combinations are also tested in this stage. Up to 3,000 people are participative in the last phase.
A fourth stage is used nowadays for post-marketing studies. This phase determines if a drug or treatment is safe over time, and is done only after all three previous phases were approved by the FDA.
Doctors conduct clinical trials before any new treatment is recommended to patients. Since mesothelioma is one of the most devastating diseases, participation in such trials has become an vital option for many malignant mesothelioma patients.
There are clinical trial reports on mesothelioma and all were aimed to raise hope for patients suffering from this disease.
Phase III Vorinostat (SAHA) trial is conducted for progressive malignant pleural mesothelioma patients. In this trial, doctors use Vorinostat, a chemotherapy drug, to kill cancerous cells, stop their division, and block enzymes that cells need in order to grow and multiply.
Phase II Pemetrexed Disodium with Gemcitabine or Carboplatin advanced malignant mesothelioma patients is a study conducted to examine the effects of chemotherapy drugs such as Gemcitabine, Pemetrexed Disodium, and Carboplatin on advanced and recurrent malignant mesothelioma.
Phase II AZD2171 is a study for patients with unresectable malignant mesothelioma. Here, doctors investigate the use of chemotherapy drugs to inhibit enzymes which slow the growth and block blood flow into the tumor. This study aims to help victims suffering from sarcotamous, advanced, epithelial, and recurrent malignant mesothelioma.
The individuals who participate in extensive clinical trials are courageous individuals who wish to take a more active role in helping mesothelioma patients. These individuals greatly contribute to a more thorough understanding of this disease and help further medical science’s options in treating mesothelioma.

Before choosing chemo as a treatment option, you should know the expected benefits, side effects, and risks. There are some questions that you should question your doctor before you start a chemotherapy treatment. The answers you will get will prepare you for the likely outcome.
These are some of the questions you should question your doctor:
-What is the goal of chemotherapy for my cancer?
-What are the chances that the chemotherapy will work?
-After chemotherapy, will I be cured, in remission, or relieved of my symptoms?
-Are there other ways to get to the same goals?
-How will I know if the chemotherapy is working?
-If the chemotherapy does not work, are there other treatments for me?
-What are the risks and side effects of the chemotherapy I will be taking? How do side effects of this chemo compare with side effects of other treatments?
-How will I get the chemotherapy, how often, and for how long?
-Where will I be given the drugs? -What can I do to get ready for treatment and decrease the chance of side effects?
-Will I need to change my diet in any way? My activities? My work? Exercise? Sexual activities?
-Will I also need surgery, radiation, or both? If so, when and why? What results can I expect from each type of treatment? -If I have chemo after surgery or radiation, will it kill any remaining cancer cells? Could chemo be used alone?
-Could I take part in a clinical trial?
-How much will chemotherapy cost? Will my health insurance cover it? -If the insurance company questions for a second opinion, or if I want to get one, can you suggest someone for me to see?
Here are some things you should do to help you remember the doctors answers:
-Take notes during your visits. Don’t feel shy about asking your doctor to slow down if you need more time to write. Question questions if you don’t know something.
-If you can, use a tape recorder during your visit so you won’t miss anything.
-Consider taking a friend or relative with you to help you know what your doctor says during your visit and to refresh your memory afterward.
Mesothelioma victims must seek as much knowledge as they can get about mesothelioma so that they can cope maximally with treatment.

Clinical trials are used to find out whether the treatment is safe for general use and whether it is really effective against diseases. Participation in clinical trials is an vital treatment option for many patients with mesothelioma as much of the currently used treatments offer small hope for a cure.
Mesothelioma is very resistant to treatment and medical experts believe that there could be an epidemic of mesothelioma cases in years to come. Clinical trials (research studies with people) are designed to find new treatments and better ways to use current treatments, raising hope for victims of this devastating disease. These are three vital ongoing clinical trials for mesothelioma treatment.
1-Phase III Vorinostat (SAHA) clinical trial for progressive or relapsed malignant pleural mesothelioma patients. This trial is studying the use of chemotherapy drug vorinostat to kill cancerous cells and stop their division, it is also believed that the drug can inhibit enzymes that are needed by the cancerous cells for growth and multiplication. These studies are currently in the third phase to know its effects on epithelial, sarcomatous, and recurrent mesothelioma. Phase III Vorinostat (SAHA) trials for progressive or relapsed malignant pleural mesothelioma patients are currently underway at the Jonsson Comprehensive Cancer Center at UCLA and Warren Grant Magnuson Clinical Center – NCI Clinical Studies Support, Bethesda, Maryland.
2-Phase II AZD2171 clinical trial for patients with unresectable malignant mesothelioma Phase II AZD2171 for patients with unresectable malignant mesothelioma is investigating the use of chemotherapy drugs to inhibit enzymes that slow the growth and block the supply of blood into the cancerous cells. Cancer cells depend on the formation of new blood vessels to bring blood that will supply the nutrients that they need for growth, new drugs are being studied that will inhibit the development of these new blood vessels. This study hopes to result in the development of drugs that will be used to treat epithelial, sarcomatous, advanced, and recurrent malignant mesothelioma. These clinical trials are underway in Arizona, Georgia, Illinois, Indiana, Iowa, Kansas, Massachusetts, Michigan, Missouri, Montana, North Carolina, Ohio, South Carolina, Tennessee, Virginia, Washington, and Wyoming.
3-Phase II clinical trial on the use of Pemetrexed Disodium with Gemcitabine or Carboplatin in advanced malignant mesothelioma patients. The Phase II clinical trial on the use of Pemetrexed Disodium with Gemcitabine or Carboplatin in advanced malignant mesothelioma patients clinical is a study to study the effects of chemotherapy drugs pemetrexed disodium, gemcitabine, and carboplatin on advanced and recurrent malignant mesothelioma. This study is in phase II and is being conducted in several locations across the country including Delaware, Florida Illinois, Indiana, Iowa, Maryland, Michigan, New Jersey, New York, Ohio, Pennsylvania, South Dakota, and Wisconsin.
Mesothelioma victims should always endeavor to question their oncologist about available clinical trials in their area.

Mesothelioma Chemotherapy – Do You Know the Schedule of Treatment and Common Side Effects?

Chemotherapy Schedules
How often you will receive chemotherapy will be determined by your doctor, taking into consideration factors such as the stage of your cancer, the types of drugs you receive, the anticipated toxicities of the drugs and the time necessary for your body to recover from these toxicities. The doctor may also consider whether the goal of the chemotherapy is to control the growth of the cancer, or to ease symptoms associated with the disease.
In general, chemotherapy treatment is administered in “cycles” – a “cycle” being defined as a period of treatment followed by a period of rest. This cycle allows the cancer cells to be attacked by the drugs, and then allows the body’s normal cells time to recover. The combination of drugs used, the length of time to administer the drugs, how often they should be repeated and the number of cycles recommended have been analyzed very well in clinical trials.
For mesothelioma patients, the “standard” treatment is a combination of Alimta and cisplatin, administered IV, with a 10 minute infusion of Alimta followed by two hours of cisplatin, given in 21 day cycles. Modifications to this schedule may be made according to what your doctor feels is appropriate in your particular case.
The number of treatment cycles, or the length of time between the beginning and end of chemotherapy may vary, but in general, 3 to 4 cycles of treatment are given before response is evaluated; 2 to 3 cycles are considered a minimum to assess for effectiveness. After response to the treatment has been determined, the following criteria will be used to choose whether chemotherapy should continue:
• If there is shrinkage of the tumor, or the disease is kept stable, chemotherapy may be continued for as long as it can be tolerated and there is no disease progression.
• If there is continued disease progression, chemotherapy will be stopped, and the patient will be given alternative options.
The goal of setting a chemotherapy schedule is to make treatment as effective, timely and distress-free as possible, but while the drugs are working to kill cancer cells, they may also affect healthy cells causing side effects.
Common Side Effects of Chemotherapy Includes:
-Fever of, or greater than 101 degrees
-Nausea
-Diarrhea or constipation
-Fatigue
-Tingling or numbness in the fingers or toes
-Ringing in the ears
-Bruises or rashes
-Sores in the mouth or throat
-Reduced white blood cell count {neutropenia} making you more prone to infections. For this reason your full blood count should be monitored closely.
Be sure you report all over-the-counter and prescribed medicines to your physician. Don’t take aspirin unless it has been approved by your physician. Question your pharmacist if aspirin is part of the drugs you want to buy.
Stay away from people who have colds or the flu. Chemotherapy can compromise your immune system and lower your resistance to germs.
It is normal to feel sad, mad or worried, but, letting these emotions get out of control can affect negatively your overall well-being. Look out for the help of family, friends, your doctor, a counselor or a support group.
Chemotherapy treatment should be a cooperative effort between you and your doctor. The interaction that takes place is vital to your health. It will not only help you feel better, but will also address any potential problems with improper communication.
Patients should educate themselves about mesothelioma and treatment options and reach out to available resources to make coping with a mesothelioma diagnosis simpler.

Mesothelioma Chemotherapy – What Are the Six Commonly Used Chemotherapy Agents?

Mesothelioma cancer, can be treated by the use of anti cancer drug therapies. These anti cancer drugs act by inhibiting tumor growth through the blocking of cell division and multiplication. This mode of action also unfortunately affects other rapidly replicating cells in the body, cells of the hair and the lining of the intestine are particularly vulnerable, thus leading to the common side effects of anti cancer drugs like hair loss and nausea.
Mesothelioma is notoriously resistant to common anti cancer chemotherapy as result of some reasons. The cancer takes a long time to mature and show symptoms, by the time these symptoms appear the tumor will be well formed with a lot of layers of dead cells that the anti cancer drugs will find it hard to penetrate, and also by the time a diagnosis of mesothelioma is made, most victims have but a small time left to live so the drugs do not have enough time to exert their maximal effect.
New drugs that have been designed to overcome these challenges and have fewer and lesser side effects include:
Alimta (Pemetrexed)
Alimta was approved by The Food and Drug Administration in February 2004 as a new anti cancer drug for the treatment of mesothelioma. Its mode of action is by inhibiting the creation of thymidine and purine , two enzymes that are vital to cell growth. This stops the mesothelioma cancer cells from multiplying at a very quick rate. Its side effects include nausea and vomiting, diarrhea, fatigue, mouth sores and low blood counts. Patients are usually placed on vitamin B12 and folic acid supplements to reduce the severity of the side effects.
Cisplatin
This is a platinum-based chemotherapy drug that prevents the reproduction of DNA and is used to treat a number of different cancers including mesothelioma. The FDA approved it in 1978 to treat a number of cancers, and has proved somewhat effective on mesothelioma. Cisplatin but has some very serious side effects which inlude ear hurt{ototoxicity} leading to hearing loss, kidney hurt, nerve hurt, and nausea, and so some oncologist are very reluctant to prescribe this drug to patients.
Tomudex (Raltitrexed)
This drug prevents cancer cells from replicating the DNA that is needed for multiplication and spread of cancer. This drug can be used as a monotherapy or in combination with onconase.
Vinorelbine
This anticancer drug was initially approved by the FDA in 1994 for the treatment of small lung cancer and breast cancer, but recent studies have shown that it could also be effective against mesothelioma, and the drug has small side effect profile. It can also be used in combination with some other chemotherapy drugs.
Onconase (Ranpirnase/P-30 Protein)
Onconase is a chemical taken from the eggs of the leopard frog that blocks protein synthesis which leads to blockage of tumor cell division and growth. It has shown promise as becoming one of the more effective drugs for treating cancer, as survival rates for one and two years are 34.4% and 21.6 respectively, as opposed to six to eight months without treatment.
Veglin
This is an anticancer chemotherapy drug that blocks the growth of tumors in people suffering from lymphoma, Kaposi’s sarcoma, and colon and lung cancers. Many oncologists believe that it could be effective in fighting mesothelioma, as Veglin inhibits vascular endothelial growth factor (VEGF), a substance that is usually found in very high concentrations in mesothelioma patients. VEGF helps tumors grow new blood vessels which help to supply nutrients to the new tumor cells. This drug inhibits this process thus starving the new cancer cells of their much needed nutrients, leading to their ultimate death.
