Pleural mesothelioma is the commonest type of mesothelioma. Mesothelioma is a rare form of cancer that attacks the mesothelial cells in the mesothelium, the tissue that forms the membranous lining of the body’s major internal organs and cavities.

Pleural mesothelioma develops when the cancer affects the pleura {the membranous lining of the lungs}.This form of cancer is commonest in the UK, followed by South Africa, then Australia, then USA, then Eastern Europe.

The pleura has two layers which gives support and protection to the lungs and chest cavity:

-The outer layer, or parietal layer, lines the entire chest cavity and the diaphragm.

-The inner layer or the visceral layer which applies directly over the lungs.

Pleural mesothelioma can start from any of these two layers then spread to the other layer. Exposure to asbestos has been strongly linked to the development of this type of cancer. The toxic fibers get trapped in the spaces between the mesothelial cells of the pleura.

Symptoms

The asbestos fibers trapped in the body cause cancerous cells to multiply and divide haphazardly without control. These results in the thickening of the pleural membrane and mesothelial cells, leading to fluid build up in the pleura {pleural effusion}. This accumulated fluid will gradually start pressing on the lungs and the respiratory system leading abnormal and hard breathing.

These developments eventually form the basis of most of the symptoms of pleural mesothelioma. These symptoms include:

A} Persistent dry or raspy cough

B} Haemoptysis, the coughing up of blood in the sputum

C} Difficulty in swallowing {dysphagia}

D} Difficulty in breathing, laboured breathing known as dyspnoea

E} Pleurisy, pain in the chest during breathing

F} Development of lumps under the skin of the chest wall

G} Night sweats or fever

H} Unexplained weight loss

I} Fatigue

Diagnosis

Just like in other forms of mesothelioma, the symptoms of pleural mesothelioma show up many years after the initial exposure to asbestos and these symptoms are not specific to pleural mesothelioma, they also occur in some other less serious lung diseases like pneumonia and influenza.

All these factors make the diagnosis of pleura mesothelioma very hard.

Once there is a strong clinical suspicion of pleural mesothelioma, diagnosis is then confirmed by imaging tests such as x-rays, CT scan, to confirm the location of the cancer. The patient is also subjected to fluid and tissue analysis known as biopsy to confirm the type of cancer involved.

Treatment

Newly developed drugs and detection techniques have improved the outlook for patients with pleural mesothelioma.

In general there are three main modes of treatment: a} Surgery b} Chemotherapy c} Radiotherapy. Most patients receive a combination of two or more of the different modes of treatment.

The earlier the cancer is detected, the better the prognosis and the wider the range of treatment options. When the tumor is detected early, curative surgery is done to remove the cancer, this is followed by chemotherapy or and radiotherapy to kill off any remnant cancerous cells. Curative surgeries is only carried out in patients that have excellent general state of health and whose cancers have not spread beyond localized sections of the lungs.

The level of progression of the tumor is classified in terms of the stage of the cancer. The stages range from stage 1 to stage 4 depending on the level of spread of the tumor.

Stage 1 and Stage 2 pleural mesothelioma have better prognosis and better treatment options.

The latter stages of the cancer, namely stage 3 and stage 4 are mostly limited to palliative modes of treatment to improve the quality of life and not their prognosis. These palliative treatments include the draining of excessive fluids in the pleural spaces and surgical removal of tumors to relieve pressure on the lungs.

According to current studies, approximately 10% of all pleural mesothelioma patients will survive for three to five years after diagnosis and 5% will survive for 5 years and above.

Over the past couple of years there has been an upsurge in interest about mesothelioma amongst the general population. The shocking truth about mesothelioma is that although Mesothelioma Cancer is a rare cancer, every year about 2,000 new cases of mesothelioma are diagnosed in the United States and this number has been continuously increasing for the past 20 years.

 Initially mesothelioma was recognized as a tumor of the pleura, peritoneum and pericardium. Later on, the doctors learned the fact link between mesothelioma cancers and asbestos exposure. J.C.Wagner was the first person to write on this link when he emphasized the linkage of mesothelioma to asbestos exposure in about 32 cases of workers in the “Asbestos Hills” in South Africa.

 

After this initial publication, the relationship between mesothelioma and asbestos exposure was confirmed in different research studies around the world. Mesothelioma is caused because of asbestos exposure and inhalation of asbestos particles. Asbestos was widely used in industrial and manufacturing processes and building materials. Construction materials such as cement, tiles, roofing shingles, pipes, insulating material and many more, contained asbestos materials. The shocking mesothelioma fact is that nearly all American buildings constructed before 1970s contain asbestos material that could pose risk for the occupants.

 

Mesothelioma Applied Research Foundation has released a fact that, though since 1970s the protections against asbestos exposure have been in place, the mesothelioma would take 30 to 50 years to be noticed. This is because mesothelioma cancer and other asbestos diseases have a high latency rate. The asbestos particles can lie dormant in the human body for 30-50 years before manifesting as mesothelioma cancers.

 

Mesothelioma cancers are of two main types. In pleural mesothelioma, the cancer cells are found in the sac lining the chest and the peritoneal mesothelioma is found in the lining of the abdominal cavity. There are rare instances of mesothelioma of the lining of hearts and testicles. Mesothelioma is more prevalent in men than in women.

 

Awareness about mesothelioma facts is also helpful in having awareness about symptoms of mesothelioma. This information helps in early detection and timely treatment of disease. Chest pain that is caused by cancer itself or shortness of breath, abdominal pain and swelling, cough, fatigue and unexplained weight loss are some possible symptoms of mesothelioma cancers.

Mesothelioma is the commonest primary tumor of the pleura. There are two main types;
1] the localized mesothelioma{bening and malignant types}
2] the diffuse malignant mesothelioma

Causes
There is a strong correlation between asbestos exposure and the development of mesothelioma, although a history of exposure is not always obtainable in patients who have mesothelioma. There are 2 types of asbestos fibers based on crystalline structure; a-serpentine {chrysolite} and b-amphibole. Included within the amphibole are amosite, tremolte, crocidolite and anthophyllite. The most harmful is the crocidolite or better known as blue asbestos.

Localised Bening Mesothelioma

These characteristically arise from the inner layer of the pleura
Symptoms and signs;
-chest pain
-shortness of breath
-most of the times there are no symptoms {asymptomatic}, these tumors are usually learned on routine chest x-ray
-hypoglycemia {low blood sugar} is sometimes associated

Treatment
Surgery is the main stay of treatment, the tumor is removed by the surgeon and recurrence is rare and when it happens transition to malignant form of tumor should be suspected.

Localized Malignant Mesothelioma
Symptoms and Signs;
-chest pain
-cough
-fever
-shortness of breath
Treatment
Surgery is also the treatment of choice, although prognosis is very poor

Diffuse Malignant Mesothelioma
This type of mesothelioma can arise form any part of the pleura and often extend into the peritoneum as well as into the ribs and chest wall {peritoneal mesothelioma and pericardial mesothelioma}. It also spreads commonly to the liver, brain and adrenal glands. Histologically malignant mesothelioma have both fibrous {sarcomatous mesothelioma} and epithelial {papillary} components.

Investigations
-CT scan of the chest is extremely useful in diagnosing the extent of the tumor.
-Needle biopsy {the surgeon inserts a special needle into the pleura to take samples of the tumor which is taken to the laboratory to see under the microscope} of the pleura is a must.
-Pleural fluid is aspirated and taken to the lab for microscopic examination.
Staging of Malignant Pleural Mesothelioma
-Stage 1:Involvement of ipsilateral pleura and lungs {affecting just one lung either left or right lung}
-Stage 2-Involvement of chest wall, mediastinum {structure at the middle of the heart}, pericardium or contralateral {opposite side} pleura
-Stage 3-Distant blood spread into other organs eg brain, adrenals

Treatment
Treatment is discouragingly ineffective and survival rarely longer than 2 years with an average of 10-14 months. In selected cases with stage 1 a radical surgery-pleuropneumonectomy may be effective, but long term survival is rare. Primary radiation therapy may be used also. Combination chemotherapy using either single agent or combination protocols have also been used but the outcome remains dismal.

Asbestos Exposure

The largest risk factor to mesothelioma is past exposure to asbestos. Around 80% of mesothelioma patients have been previously exposed to asbestos. This is the reason why some people alternatively call mesothelioma asbestos cancer. There could be a lot of ways in which exposure may happen. One of the most common ways but is through work situations. Employees who work for companies that use asbestos are the most at risk of developing mesothelioma.

There are but some patients who claim that they never have been exposed. There may therefore be some other risk factors involved. There are also a number of patients who may not exactly recall when and where they were exposed to asbestos. This is especially right for people who have indirect asbestos contact. The fibers of asbestos for example can be inhaled or ingested from objects like the clothes of people working with asbestos.

Appearance of Symptoms

Early treatment is often the key to increasing cancer patient survival rates. It is possible that the same can be right for mesothelioma. Many asbestos cancer patients but only learn of their illness when it is at an advanced stage. This is because the symptoms of mesothelioma seem a lot like the symptoms of other diseases. Mesothelioma symptoms can therefore be a bit misleading. Moreover, the symptoms may appear only after 20-30 years after exposure. By that time many patients do not expect it and may even forget that they have had some experience with asbestos.

Litigation

Asbestos and mesothelioma are also often heard together in court rooms. This is because an asbestos cancer patient may have developed the illness due to company negligence. It has been proven in decades ancient cases that some companies were really aware of the dangers of asbestos and did not take steps to protect their employees. In recent cases, it may still be possible for some companies to have been negligent despite existing asbestos handling laws.

If you have reason to reckon that a company has been negligent, you can sue. It is possible that you were not warned about asbestos or a company’s precautionary measures were not up to standard.

Prognosis

On the average, mesothelioma prognosis is not excellent. Based on average statistical data, a patient may not live beyond one year. This is not but an absolute figure. There have been some patients who have lived for more than five years. It is vital to remember that your particular condition may not be the same as other people’s conditions. Your hopes of survival are affected by a lot of different personal factors.

Asbestos and mesothelioma truly are interconnected. If you have reason to believe that you have been exposed to asbestos, you may want to question your doctor about monitoring.

Mesothelioma: Facts About Mesothelioma

Mesothelioma is a form of cancer that affects the mesothelial cells of the body. These are the cells that make up the outer lining for the body”s major organs, such as the heart, lungs and stomach. These linings are referred to as the mesothelium and this is how the cancer got its name.

Pleural Mesothelioma

The pleura is the tissue that covers and lines the lungs. These are referred to by the medical community as pleural membranes. The pleura are fibrous membranes and the space between them is the pleural space. The pleural protect the lungs by producing a lubricant that fills the pleural space. This lubricant also allows the lungs to go easily within the chest cavity as we inhale and exhale.

Pleural mesothelioma is the most common type of the disease. Since the lungs are so close to the heart, it is nearly always affected. The pericardium is the lining found on the outside of the heart and allows it to go freely within the heart cavity.

The Peritoneum

The peritoneum is the tissue lining the abdomen. Its job is to protect the abdomen”s contents. It produces a fluid that acts as a lubricant so organs within the abdomen may go freely. Peritoneal mesothelioma is cancer of the tissue that lines the abdominal cavity. This form of mesothelioma is more rare than pleural mesothelioma.

Causes of Mesothelioma

Mesothelioma is caused by unprotected exposure to asbestos and affects those who were place at risk for the last 50 years.

Who Gets Mesothelioma

The disease is most common in males who are between the ages of 60 and 70 years ancient. These men were constantly exposed to asbestos dust and fiber, which caused the mutation of the mesothelioma cells. Mesothelioma takes years to develop, which means early diagnosis is nearly impossible.

Others at risk for mesothelioma are those who lived in the same household with someone who was constantly exposed to asbestos. Men carried asbestos dust and fibers into their homes on clothing. It was then breathed in by family members. This place them at risk of contacting mesothelioma and other diseases related to asbestos years after the fact.

Treatment

The success of treatment for mesothelioma isn”t high. The final stages of the disease are fatal. The earlier mesothelioma is diagnosed, the better the prognosis.

Mesothelioma is a cancer that resides in the lungs and is associated with asbestos. Malignant Mesothelioma is a very rare form of cancer. These cancer cells are found either in the lining around the heart,abdominal cavity or the lungs.

Mesothelioma takes years before it becomes cancerous. People from the 1940 and up are just now being diagnosed with Mesothelioma.

Asbestos is the leading cause of Mesothelioma. During world war two the manufactoring of asbestos became more increased due to the fact that the lumber was being used for building ships. Wood was in fantastic demand by the government and companies needed a replacement for their businesses, like construction, automobile factories, building trades, etc. The people who manufactored asbestos knew that asbestos wasnt a regulated product and knew that their workers would inevtably suffer from its consequences. But lumber was hard to find during this peroid and asbestos could be manufactored for much cheaper, thats why it has been in use after world war two and through up to the 1970′s. During the 1970′s asbestos was outlawed to use by the Consumer Products Safety commission in products such as wallboard patching material and artificial ash for gas fireplaces because the fiber in these products would become airborn during use.

Most people that have been diagnosed with Mesothelioma have been workers who were directly exposed to asbestos, but a few have been diagnosed with Mesothelioma from just living with the workers. The workers would have the fibers of asbestos in their hair and on thier clothing, which would travel in the air and settle in the lungs of whoever came in contact with them.

As with all cancer, the earliest detection of Mesothelioma will give better opportunity on its treatment.

Chest pain, shortness of breath and a persistant cough are some of the early symptoms of Mesothelioma. This is a very hard problem to find, because it sometimes resembles viral pneumonia. Fever, night sweats and weight loss are some of the less common symptoms of Mesothelioma.

Mesothelioma Survival Rate Facts

Mesothelioma is a honestly new type of cancer that is very serious and life threatening, as with most types of cancer.  One thing that makes mesothelioma such a serious condition is that it is usually not found until the late stages of the cancer, causing the survival rate to be low.  This is because the symptoms don’t usually arise until the late stages. 

By this time the patient most likely doesn’t have much longer to live.  Studies show most patients who are diagnosed with mesothelioma have a year or less left to live.  Along with diagnosis being hard and delayed, mesothelioma is also honestly uncommon and we are still learning more about the disease.

But, some studies show that one out of every ten mesothelioma patients will live up to three years after diagnosis and one out of every twenty people will live five years after diagnosis. 

Studies are performed daily and new clinical trials are helping to increase the survival rate of those with mesothelioma. Survival rates vary depending on where the study is performed, and who is performing the study.  In general, studies are very close in comparison.

Survival rates are based on several different factors.  Some of these factors are, the size of the tumor, the stage the patient is in, whether the tumor can be completely removed with surgery, the quantity of fluid in the chest and abdomen and how long it took to diagnose the cancer. 

Some other fascinating facts about mesothelioma survival rates are that the average age of the patients who are diagnosed with mesothelioma are sixty one years of age and eighty one percent were male.

As said before, many studies are performed daily to find new ways to prolong the survival rate of those who have mesothelioma.  Many studies have already proven to do just that.  Lots of studies show that chemotherapy greatly increases the survival rate of those with mesothelioma.  So, although this is a newer and less common cancer we are learning more about it daily. 

With our learning we’ll eventually come up with more options of treatment and this will lead to an all together higher survival rate.
